    Job Description Summary

    Responsible for the daily operations of business line by delivering complete client satisfaction and the highest caliber of care service. The Intake /outpatient Coordinator is responsible for assisting in the admission process for outpatients. This includes establishing relationships with potential patients and/or their POA’s thoroughly explain insurance coverage and/or financial responsibility, accurately monitor provider's documentation signatures for compliance, and accurately maintaining medical records in appropriate EMR.

    Essential Job Functions/Responsibilities

    • Electronically contact providers for OPT therapy orders.
    • Accurately track OPT documents for signatures and compliance.
    • Notify DOR when documents exceed 30-day compliance regulations.
    • Accurately creates, sends, and monitors delayed certifications for compliance regulations.
    • Contacts patients and/or POA for verbal consent of OPT therapy and accurately reviews insurance eligibility and financial responsibility.
    • Accurately completes OPT consents and transmits to POA or evaluating clinician for signature.
    • Effectively track and monitor completion of consents and correctly updates medical records in appropriate EMR.
    • Proficiency in multiple EMR systems and Microsoft applications.
    • Communicate effectively with field clinicians for information gathering purposes.
    • Responsible for ordering DME for patients as per clinician recommendation by obtaining MD order and appropriate documentation for submission to provider.
    • Acts as a liaison between DOR and patients/families by effectively addressing questions and/or concerns.
    • Work with Community Liaison and Rehab Coordinator to ensure all required referral documents are expedited for valid referral.
    • Confers with the DOR as to the preliminary eligibility for services.
    • Participates in company quality improvement activities.
    • Understands and adheres to established policies and procedures of agency, as well as compliance to local state and federal laws.
    • Filing all elements of patient records and monitoring date on all signed documents to ensure accuracy and completion in a timely basis.
    • Maintaining adequate supply of patient clinical record elements and stock of premade clinical records available for clinicians.
    • Answers telephone lines promptly, efficiently and channel them appropriately.

    Position Qualifications

    • High school diploma or equivalent required
    • Excellent working knowledge of primary home health diagnosis, related interventions, and care protocols.
    • Demonstratable working knowledge of Medicare and non-Medicare reimbursement requirements including eligibility for service. Managed care and VA pre-authorization processes and Medicare/managed care outpatient therapy services requirements.
    • Demonstrates good communication and public relations skills.
    • Passionate for delivering a world class customer experience.
    • Must be able to travel if indicated for training and/or Discovery Communities/Properties.
    • Proficient in Microsoft Office applications.
    • Basic knowledge of OPT therapy processes and CMS guidelines.
