Position Summary
- Community Liaison Entry Level
- Maintains an in-depth knowledge of the community, health care facilities, and referral sources appropriate for agency services.
- Assists the Marketing and Business Development Leadership team in establishing targets and goals for the development of ALL service lines in the agency.
- Responsible for establishing, growing, and maintaining relationships with facility-based referral sources, in accordance with Company policies and procedures, by both communicating with existing referral sources and identifying new opportunities.
- Works with new clients and families on current and future needs for care and obtains consents and contracts as requested.
- Remain current on HHA education and competencies to assist with patient care as needed.
- Provides in-services and professional presentations for referral sources, community groups and other organizations, regarding DAH’s services, programs, and outcomes.
- Maintains regular contact with building partners, including but not limited to DHW, ED’s, Activity Directors, Resident Service Directors, and care staff.
- Assesses the patient’s clinical needs and gathers clinical information from the hospital, ALF, family, and the patient and provides to intake department.
- Communicates accurately and completely to physicians, staff members, patients, and families, utilizing positive interpersonal skills.
- Completes all required documentation on correct forms with accuracy, timeliness, legibility, and professional terminology.
- Champions the mission and vision of the organization and projects a positive and professional image for the agency in the community.
- Assist with tasks and activities as assigned by Directors or Administration.
- Attends all patient care conferences as scheduled.
- Develops and maintains a retention/wellness program for team members within the communities served.
Qualifications:
- Must understand healthcare operations, legal guidelines, competitive analysis, and marketplace trends.
- Must have reliable transportation, valid Florida driver's license and be able to provide proof of automobile insurance.
- Experience in a home care and/or health-related field preferred.
- Ability to set priorities; competent organizational skills.
- Ability to handle stressful situations/deadlines.
- Excellent oral/written communication, presentation, and interpersonal skills.
- Must demonstrate the ability to communicate effectively with all members of the health care team.
- Demonstrates desire to work in a business development role.
